Notice: Required Office Forms & Credit Card Processing Fee
Effective March 1, 2025 we have updated our office policy. Please review and complete the appropriate forms to be in compliance with our patient policy.
Effective April 15, 2025 there will be a 2% credit card processing fee added to any charges. We will now be accepting American Express.
All portal messages will be responded to by the appropriate Shine team member on Tuesdays and Fridays ONLY by end of business day. Please choose one means of communication, either portal message or call the office directly. If you are experiencing an emergency, call 911 or have someone take you to the nearest emergency room.
